如果胆囊发炎极重或有巨大的结石时，就应该考虑进行胆囊的开放性手术（即剖腹手术）。手术切口会在肝区的腹壁上，当打开腹腔展露肝脏后，将肝脏拔开，把连向胆囊的胆管、血管断开，最后移除胆囊。然后检测胆总管是否通畅（肝与小肠相连的胆管）。术后几天内会在病灶附近保留一条引流管，以引流术后的渗液。</p>

大多数患者在腹腔镜手术后的当天或第二天便能离院回家，并可进行正常的饮食或进行比较剧烈的运动。进行开放性手术的病人，多数需住院观察5-7天，一周后便可恢复正常的饮食。4-6周后可恢复正常的运动。</p>
