这个冬天真暖和



我市铜梁县农村妇女杨某，年仅38岁，与丈夫、孩子，抱着满腔热情，投身广东打工潮，意欲“淘金”改善贫穷面貌。可事与愿违，2007年10月左右，患者突然觉得右上腹胀痛，在广东某医院诊断为原发性肝癌；由于肿瘤巨大，当地多家医院均不愿对其进行手术治疗。在绝望之余，夫妇俩想到了家乡，想到了一直以来信誉颇好的武警重庆总队医院肝胆胰脾外科中心。

    当夫妇俩怀着惴惴不安的心情来到该中心钟彦文主任的诊室，受到了热情接待。当钟主任查看患者携带的院外CT片子后，发现该患者右肝叶巨大占位性病变约18.5cm×12.5cm×12cm大小（图1），整个正常右肝的位置被巨大肿瘤占据，周围紧邻的脏器和大血管被挤向左、向上、下。其中，右肾下移达12Cm，而肿瘤内侧紧靠的肝脏大血管受压移位变形，肿瘤上方顶着膈肌突入胸腔。从CT上看，肿瘤与周围组织器官没有明显界限，手术难度与风险非常高。

看着钟主任紧锁的眉头，杨某的丈夫额头汗水都浸出来了，而杨某本人一脸的疲惫，用绝望的眼神望着两鬓斑白的钟主任。都20分钟过去了，钟主任还在查看CT片，杨某实在忍不住了，用颤抖的声音问道：“教授，有希望吗？”

而这时的钟主任，尽管犹豫、担心，但最后凭着多年的经验，告诉患者：肿瘤尚有包膜，但肿瘤太大，手术风险很大，估计能够切除，还有希望。

    经过一周多的纠正贫血等术前准备，2007年11月21日上午9：00，钟彦文主任带着陈俊主治医师、邓开主治医师、李广阔主治医师 等三位助手，开始了紧张的右肝叶切除术。经右侧胸腹联合切口，入腹腔探查发现，肿瘤非常巨大，仅能触及肝下缘露出的肿瘤“冰山一角”（图2），肝脏向左侧转位，右肾下移，肿瘤无法推动。鉴于手术操作空间十分小，且肿瘤滋养血管丰富，很容易出血；加之肿瘤紧邻下腔静脉、门静脉等大血管，如不慎撕破大出血，后果……。但钟主任等仍然凭借娴熟的外科手术技巧，在狭小的空间中分离、钳夹、止血、结扎……，上数百次的打结，磨破了好几双手套，终于，下午5：00。钟主任和盘托出了整个肝脏肿瘤。经标本解剖，肿瘤大小约20cm×13cm×12.5cm、重量约2Kg左右，肿瘤中心部位由于缺血坏死，呈烂棉絮状(图3)。晚上9：00，手术结束，医生们拖着疲惫的身躯走出了手术室。看着微睁眼睛、睡意朦胧的妻子，患者杨某的丈夫露出欣慰笑容。

患者术后经过医生们的精心治疗近1月后，CT复查表明，患者肝脏组织复位良好（图4），肝功能良好，顺利出院。出院时，患者及家属给医生和护士们送来了一面锦旗，“巧手除病魔、深情赛春天”。

这个冬天还真不冷！
